Important Appointment Reminders

1. Schedule your appointment above by pressing "Book" for the desired service.
2. Receive an e-mail with required forms and consents to be filled prior to the scheduled appointment. 
3. If your visit requires a video visit, on the day of the appointment, you will receive a text message with the link for your virtual visit. No application download is required. You may attend the telemedicine session via any computer, tablet or telephone that is camera and microphone enabled. 
4. All other services-you will receive a confirmation text one day before and the day of your appointment. We know things happen, if you are unable to make the appointment please cancel 24 hours before. 
5. Your credit card will be charged the total amount agreed upon between you and the provider. 
6. Your provider will send any prescriptions or lab orders following the visit, if indicated. 

Resilient Minds Behavioral Health would like to kindly remind our patients of the following rules, requests, and service limitations. Your appointment time has been exclusively set aside for your mental health growth and development, and we want to ensure that you have the best experience with your mental health nurse practitioner.

• Always conduct your session from a quiet and confidential place in your home or office, where you will not be distracted during your appointment.
• Make sure you have access to a high-speed Wi-Fi connection during your virtual visit to avoid dropped sessions.
• All appointments must be booked online.
• We require credit card payment upon booking of appointment.
• We are only accepting patients over the age of 18 at this time.
• All medical history and consent forms must be completed at least 24 hours in advance of your scheduled appointment time. If patient disregards this policy, appointment cancellation, rescheduling, and/or additional fees may apply.
• If you arrive more than 7 minutes late for your appointment, the visit will need to be rescheduled and additional fees will apply. We understand emergencies and extenuating circumstances may occur from time to time, and we will consider these situations on a case-by-case basis.
• We DO NOT conduct appointments while patients are in public settings or while they are driving.

• Clients under the age of 18
• Absence of legal guardian in case of dependent adult
• Inability to consent to treatment
• Active psychosis with agitation
• Actively suicidal or homicidal clients*
• Clients arriving intoxicated
• Intellectual disorders
• Severe Neurodevelopmental disorders

* If you or someone you know is experiencing suicidal thoughts, visit your local emergency room or call the National Suicide Prevention Lifeline at 1-800-273-8255.

 If patients demonstrate a consistent pattern of late appointment arrivals or cancelled/rescheduled appointments, these patients may be denied further services, per the discretion of our practice.

Resilient Minds Behavioral Health reserves the right to discharge patients from our practice after 3-4 violations pertaining to no shows, late cancellations, or rescheduled appointments. Extenuating circumstances may be taken into consideration on a case-by-case basis.

If you are unable to make your appointment please cancel 24 hours in advance or you will be charged 50% of the service cost, no exceptions. There will be no refunds for any services provided.

As standard policy, we only provide medication refills during scheduled appointments. However, we do understand that unforeseen circumstances happen, and that patients may occasionally find themselves running out of medications before their next appointment. To prevent this situation, we strongly encourage our patients to proactively manage their prescriptions and avoid interruptions in care by making timely follow-up appointments and/or arriving on time for all scheduled appointments.

In the event of an urgent refill need, we do charge a fee to provide a bridge supply of medications, which factors in the additional time needed to review your chart and make a proper clinical decision.

 – $35.00 to change a pharmacy after a controlled medication script has been electronically completed.
– $35.00 to provide a bridge supply of non-controlled medications up to the next appointment.
– $100.00 for a 30-day script of a controlled medication in the case of missed appointment or untimely booking of a follow-up visit.

Please allow up to 24 hours for such requests to be completed.
